Natalizumab
Indication
Monitoring of the Natalizumab level and control of Anti-Drug Antibodies (ADA) against the drug.
Method
Concentration determination is performed with ELISA, in serum.
ADA analysis is performed with RIA, in serum.
Result
Drug concentration is reported as a concentration (µg/mL).
Measurement range: 0.025 – 3277 µg/mL.
ADA is reported as a concentration (AU/mL). ADA <11 AU/mL are not detectable, 11-20 AU/mL are detectable but not quantifiable and >20 AU/mL can be quantified.
Interpretation
The sample should be drawn at trough value. Low or unmeasurable concentration of the biologic pharmaceutical is associated with poor clinical effect. One explanation can be that antibodies against the drug (anti-drug antibodies, ADA) block the effect and enhance clearance of the drug. Presence of drug in the test sample may prevent the detection of ADA.

Metod
Koncentrationsbestämning utförs med ELISA i serum.

Läkemedelskoncentration anges som koncentration (µg/mL). Mätområde: 0.025 – 3277µg/mL
ADA anges som koncentration (AU/mL). ADA <11 AU/mL kan inte detekteras, 11-20 AU/mL kan detekteras men inte kvantifieras och >20 AU/mL kan kvantifieras.
Tolkning
Provet bör tas vid dalvärde. Låg eller omätbar koncentration av biologiskt läkemedel är associerat med sämre klinisk effekt. En viktig orsak kan vara att antikroppar mot läkemedlet (anti-läkemedelsantikroppar, ADA) har bildats och blockerar dess effekt och påskyndar clearance av läkemedlet. Om läkemedlet kan påvisas i provet kan ADA inte säkert detekteras.
